The providers that you have come to know and trust are still here.
I HAVE TO GIVE YOUR NPI OR TAX ID NUMBER TO MY INSURANCE. DID THAT CHANGE?
No. Both remain the same.
I JUST RECEIVED A BILLING STATEMENT FROM YOU AND MY INSURANCE. IT HAS THE OLD NAME ON IT.
During the transition period you will see the old name on a variety of documents, including on your Explanation of Benefits (EOB).
I HAVE TO PAY MY BALANCE. WHO DO I WRITE THE CHECK OUT TO?
